Surrey Downs Health and Care, delivered by Epsom and St Helier University Hospitals NHS Trust in Dorking, is seeking a part-time Care Co-Ordinator to work with GPs, paramedics, pharmacists and district nurses to coordinate and navigate care for our population.

You will take referrals, identify those who could benefit, support patients and families, and help them access appropriate services. Join a growing team and contribute to a one-team approach delivering person-centred care in the community.

Aufgaben

• Make appointments for patients and coordinate across primary, secondary, community and social services.
• Help people transition between secondary and community care, with follow-up and navigation.
• Identify when action or support is needed, alert clinician and safety concerns.
• Keep accurate records in GP and other relevant records systems, adhering to governance.
