Job summary

Surrey Downs Health & Care is recruiting a Band 5 Nurse to work with Cobham and Oxshott PCN in Leatherhead. You will assess care needs and develop holistic care plans within the community, driving locally to visit patients and coordinate with the team.

The post offers part-time hours (22.5 per week), with weekend work on a 1-in-4 rota, and requires current UK driving licence and vehicle access. DBS checks and safeguarding responsibilities apply.

Qualifications

• Registered Adult Nurse with NMC
• Valid Driving Licence (to drive within UK)
• Enhanced DBS check required for roles with safeguarding responsibilities

Responsibilities

• Work as part of the team undertaking lead responsibilities as negotiated and agreed with District Nurse
• Assess individual care needs, implement and evaluate holistic programmes of care
• Be responsible for maintain data entry and other record keeping
• Be aware of and act upon procedures that protect vulnerable individuals
