Zusammenfassung

Surrey Downs Health & Care is recruiting for a part-time role at the Medwyn Centre, Dorking. You will work alongside GPs, district nursing staff and care coordinators to coordinate and navigate patient care within a ‘one team’ approach.

You will take referrals, proactively identify patients who could benefit, and provide practical advice to improve health and wellbeing while supporting carers and families. DBS checks are required for safeguarding vulnerable groups.

Qualifikationen

• Car Driving Licence is required.
• NVQ Level 3 in adult care or equivalent qualifications or working towards.
• Intermediate IT skills with Outlook, Word, Excel and PowerPoint.

Aufgaben

• Take referrals and identify people who could benefit from care co-ordination.
• Have empathetic conversations with patients and family to understand needs.
• Support patients to understand and navigate health and wellbeing resources.
• Keep accurate records and use GP and other systems in line with governance.
