Job description

We are looking to recruit an enthusiastic pharmacist to join our great pharmacy team of two pharmacists (one prescribing), two pharmacy technicians and three prescription clerks. This position is for 18hrs a week and we are happy to be flexible about how you want to spread this over the week. This position would suit a pharmacist who is looking to develop their prescribing career within Primary Care. The practice already has a well established pharmacy team who would be able to help develop your skills and offer support and guidance which will enable the right candidate to progress both personally and professionally. Ideally we would like the candidate to start as soon as possible.

Main duties of the job

The pharmacist willwork as part of the General Practice team to bring pharmacist based skills tothe primary care team. The role willinvolve direct patient contact with clinics for structured medication reviews (SMRs);clinics in specialist area of choice working towards a prescribingqualification; mentoring pharmacy technicians; medication queries from staff,patients, pharmacies and hospital; medication optimisation in line with the IntegratedCare Board (ICB) and national requirements and support the practice withQuality and Outcomes Framework (QOF), locally comissioned services (LCS),enhanced services (ES) and Care Quality Commission (CQC) searches.

Job responsibilities
  • Work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas
  • Work independently as a prescribing pharmacist
  • Be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially older people, and those with multiple co-morbidities
  • Provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients and to help in tackling inequalities
  • Provide leadership on person-centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to QOF and ES.
  • Through structured medication reviews, support patients to take their medications to get the best from them, reduce waste and promote self care.
Person Specification
Qualifications
  • Completed an undergraduate degree in Pharmacy
  • Registered with the General Pharmaceutical Council
  • Working towards or holding a prescribing qualification
  • Prescribing in hypertension and/or hyperlipidaemia
Experience
  • Have a minimum of two years post graduate experience
  • Experience of working in primary care
  • Experience of working with GP practice systems (EMIS, Docman and AccuRx)
