Clinical Pharmacist – Streatham, South London

Location: Streatham, South London
Contract: Full-time or Part-time (flexible working considered)

An established and highly regarded training general practice in Streatham is seeking an enthusiastic and dedicated Clinical Pharmacist to join its growing multidisciplinary team. The Role

The Clinical Pharmacist will play a key role in supporting the practice’s clinical services and medicines optimisation agenda. Working closely with GPs, nurses, and other allied healthcare professionals, the post holder will help ensure safe, effective, and evidence-based prescribing across the practice population.

Key responsibilities include, but are not limited to:

  • Undertaking structured medication reviews, including for patients with complex polypharmacy
  • Supporting the management of long-term conditions in accordance with local and national guidelines
  • Medicines reconciliation following hospital discharge and outpatient appointments
  • Addressing prescribing queries and providing expert medicines advice to clinicians and patients
  • Supporting prescribing safety, quality improvement, and audit activity
  • Reducing medicines-related risk and improving cost-effective prescribing
  • Contributing to patient education and shared decision-making
  • Supporting PCN and practice-wide clinical initiatives

There will be opportunities to develop special interests aligned with the pharmacist’s experience and the needs of the practice.

The Ideal Candidate

The successful applicant will be:

  • A GPhC-registered Clinical Pharmacist
  • Experienced in primary care or keen to transition into general practice

  • A strong communicator with the ability to work effectively within a multidisciplinary team
  • Committed to continuous professional development and reflective practice
