Care Team Lead
As a Team Lead, you will have excellent communication, organisation and planning skills. You will oversee the delivery of care within Cardiff and the Vale of Glamorgan; responsible for managing the care staff, defining care packages, assessing risks and ensuring that the high quality care delivery complies with care regulations.
Working with a care coordinator, you will select suitable care workers for your area, design their rosters and ensure our clients are happy with the care that's delivered. Depending on the hours of care to be provided, you will be supported by a senior carer to help you run the team, deliver quality care and support care staff to be high performers in their jobs.
Your role will also play a key part in social care and health working more closely together. You will be expected to work alongside allied healthcare professionals such as GPs, District Nurses, Pharmacists, and other community services to ensure our common clients have good health outcomes. Yours will be a strong voice in representing the views of social care in the delivery of the client’s care.
You, your senior carers and care coordinator will form part of the on-call system which is supported by the use of a mobile rostering and care management system which can be used in office or in the field.
We will support your career development but we expect you to have a minimum Level 3 in Health and Social Care qualification and at least 2 years supervisory experience, preferably in the social care or allied health care sectors.
