Job summary

Active Care Group is seeking an experienced Complex Care Support Worker to join our Care Response Specialist team. You’ll travel within London and surrounding areas delivering urgent, person-centred care to complex needs, helping patients leave hospital safely and stabilise care packages.

This regional role offers paid mileage, a £3,000 annual car allowance, and a supportive clinical team. If you’re calm under pressure, capable of mentoring colleagues, and committed to making a real difference,

Qualifications

• Full UK Driving Licence and willingness to travel regionally.
• Experience supporting people with complex care needs.
• Ability to work independently in varied environments.
• Strong communication and organisational skills.

Responsibilities

• Support safe and timely hospital discharges.
• Deliver high-quality, person-centred care.
• Support clients with daily living activities and promoting independence.
• Build routines that improve confidence and wellbeing.
• Provide reassurance and continuity for clients and their families.
• Mentor and support new Support Workers.
• Work alongside Care Managers and Clinical Professionals.
• Ensure medication, equipment and care supplies are in place.
• Identify concerns and escalate appropriately.

If you're an experienced Complex Care Support Worker who thrives on variety, enjoys working independently and wants to make a real difference when it matters most, we'd love to hear from you. As a Care Response Specialist, you'll be part of a highly skilled team providing urgent support wherever it's needed across your assigned region. Every assignment is different. One week you could be helping someone leave hospital safely and settle back into their own home. The next, you could be stepping in to stabilise a complex care package, preventing a hospital admission and giving a family much-needed reassurance. This is a role where your experience, compassion and confidence genuinely change lives.

This is a regional role so you will need to be happy and able to travel across your designated region. If you're looking for a role based in one local area, this position is unlikely to be the right fit. Essential:

• Full UK Driving Licence
• Experience supporting people with complex care needs
• Confidence working independently and adapting to new environments

• Flexibility to travel regionally

Desirable:

• Hospital discharge experience
• Level 2 Health & Social Care qualification (or above)
