Job summary

Active Care Group in Manchester is seeking a Care Response Specialist to join our regional team. You will coordinate rapid complex care responses, support hospital discharges, and deliver person-centred care in clients' homes.

With a Full UK Driving Licence and the ability to travel regionally, you will mentor new Support Workers, collaborate with Care Managers and clinicians, and ensure medications, equipment and supplies are in place to keep people safe at home.

Qualifications

• Full UK Driving Licence is essential.
• Experience supporting people with complex care needs.
• Ability to work independently in diverse settings.
• Excellent communication and organisational skills.
• Calm, compassionate and professional approach.
• Willingness to travel regionally.

Responsibilities

• Support set-up, stabilisation and delivery of complex care packages.
• Assist with safe and timely hospital discharges.
• Provide high-quality, person-centred care.
• Support daily living activities and promote independence.
• Build routines to improve confidence and wellbeing.
• Provide reassurance and continuity for clients and families.
• Mentor and support new Support Workers and collaborate with Care Managers and clinicians.
• Ensure medication, equipment and care supplies are in place and escalate concerns.
