Job summary

Active Care Group in South Wales is seeking an experienced Care Response Specialist to support urgent, complex care packages across your region. You will help with hospital discharges, deliver high-quality, person-centred care, and mentor new Support Workers while travelling regionally.

You will work with a caring, experienced clinical and operational team to ensure each client receives consistent, safe care and support.

Responsibilities

• Support safe hospital discharges and complex care packages.

If you’re an experienced Complex Care Support Worker who thrives on variety, enjoys working independently and wants to make a real difference when it matters most, we’d love to hear from you. As a Care Response Specialist, you’ll be part of a highly skilled team providing urgent support wherever it’s needed across your assigned region. Every assignment is different. One week you could be helping someone leave hospital safely and settle back into their own home. The next, you could be stepping in to stabilise a complex care package, preventing a hospital admission and giving a family much-needed reassurance. This is a role where your experience, compassion and confidence genuinely change lives.

You’ll support the set-up, stabilisation and delivery of complex care packages, including:

• Supporting safe and timely hospital discharges
• Delivering high-quality, person-centred care
• Supporting clients with daily living activities and promoting independence
• Building routines that improve confidence and wellbeing
• Providing reassurance and continuity for clients and their families
• Mentoring and supporting new Support Workers
• Working alongside Care Managers and Clinical Professionals
• Ensuring medication, equipment and care supplies are in place
• Identifying concerns and escalating appropriately

Travel is an essential part of this role:

This is a regional role so you will need to be happy and able to travel across your designated region If you’re looking for a role based in one local area, this position is unlikely to be the right fit.
