Active Care Group is seeking a Care Response Specialist for North Wales. You will support safe hospital discharges, deliver person-centred care, and assist clients with daily living, while travelling regionally across North Wales.

You will mentor colleagues, collaborate with care managers, and ensure medications and equipment are in place. A Full UK Driving Licence and flexibility to travel are essential.
