Job summary

Active Care Group is seeking a Care Response Specialist in Birmingham. The role involves supporting safe hospital discharges, delivering high-quality, person-centred care, and assisting clients with daily living activities across your region.

You will collaborate with Care Managers and Clinical Professionals, mentor junior staff, and manage equipment and supplies while ensuring continuity of care for families. A full UK driving licence and independence are essential.

Qualifications

  • Full UK Driving Licence is essential.
  • Experience supporting people with complex care needs.
  • Confidence working independently and adapting to new environments.
  • Excellent communication and organisational skills.
  • Desirable: Hospital discharge experience.
  • Desirable: Level 2 Health & Social Care qualification (or above).

Responsibilities

  • Support safe and timely hospital discharges.
  • Deliver high-quality, person-centred care.
  • Assist clients with daily living activities and promote independence.
  • Build routines to improve confidence and wellbeing.
  • Provide reassurance and continuity for clients and families.
  • Mentor and support new Support Workers.
  • Work alongside Care Managers and Clinical Professionals.
  • Ensure medication, equipment and care supplies are in place.
  • Identify concerns and escalate appropriately.

One week you could be helping someone leave hospital safely and settle back into their own home.
