Job summary

Active Care Group is seeking an experienced Care Response Specialist to join our regional team. You’ll travel across Newcastle and surrounding areas to support urgent, complex care packages and help clients leave hospital safely or stay at home.

You’ll work independently within a skilled clinical team, mentoring new Support Workers and coordinating with Care Managers and Clinical Professionals to ensure timely discharges, continuity of care, and person-centred support.

Qualifications

• Full UK Driving Licence required.
• Experience supporting people with complex care needs.
• Ability to work independently in diverse environments.
• Strong communication and organisational skills.
• Calm, compassionate and professional approach.
• Flexibility to travel regionally.

Responsibilities

• Support set-up, stabilisation and delivery of complex care packages.
• Assist safe and timely hospital discharges.
• Provide high-quality, person-centred care and daily living support.
• Build routines to improve confidence and wellbeing.
• Mentor and support new Support Workers.
• Collaborate with Care Managers and Clinical Professionals.
• Ensure medication, equipment and care supplies are in place and escalate concerns when needed.

As a Care Response Specialist, you’ll be part of a highly skilled team providing urgent support wherever it’s needed across your assigned region. Every assignment is different. One week you could be helping someone leave hospital safely and settle back into their own home. The next, you could be stepping in to stabilise a complex care package, preventing a hospital admission and giving a family much-needed reassurance. This is a role where your experience, compassion and confidence genuinely change lives.

Travel is an essential part of this role: This is a regional role so you will need to be happy and able to travel across your designated region If you’re looking for a role based in one local area, this position is unlikely to be the right fit.
