Job summary

Active Care Group is seeking an experienced Complex Care Support Worker to join our Care Response Specialist team across the Newcastle region. You’ll deliver urgent, person-centred care, support safe hospital discharges and help stabilise complex care packages wherever you’re needed most.

This regional role requires driving, independence and compassion. You’ll earn up to £29,000 per year (DOE) plus a £3,000 annual car allowance and paid mileage.

Qualifications

• Full UK Driving Licence.
• Experience supporting people with complex care needs.
• Confidence working independently and adapting to new environments.
• Excellent communication and organisational skills.
• A calm, compassionate and professional approach.
• Flexibility to travel regionally.
• Desirable: Hospital discharge experience.

Responsibilities

• Set-up, stabilisation and delivery of complex care packages.
• Support safe and timely hospital discharges.
• Deliver high-quality, person-centred care.
• Support daily living activities and promote independence.
• Build routines that improve confidence and wellbeing.
• Provide reassurance and continuity for clients and families.
• Mentor and support new Support Workers.
• Work with Care Managers and Clinical Professionals.
• Ensure medication, equipment and care supplies are in place.
• Identify concerns and escalate appropriately.

As a Care Response Specialist, you’ll be part of a highly skilled team providing urgent support wherever it’s needed across your assigned region. Every assignment is different. One week you could be helping someone leave hospital safely and settle back into their own home. The next, you could be stepping in to stabilise a complex care package, preventing a hospital admission and giving a family much-needed reassurance. This is a role where your experience, compassion and confidence genuinely change lives.

Travel is an essential part of this role:

This is a regional role so you will need to be happy and able to travel across your designated region. If you’re looking for a role based in one local area, this position is unlikely to be the right fit.
