Job summary

Active Care Group is seeking a dedicated Support Worker to help set up, stabilise and deliver complex care packages across London and surrounding areas.

You’ll support safe hospital discharges, provide high-quality person-centred care, assist with daily living activities and build routines to boost confidence and wellbeing, while mentoring new staff and working with Care Managers and Clinical Professionals.

Qualifications

• Full UK Driving Licence.
• Experience supporting people with complex care needs.
• Confidence working independently and adapting to new environments.
• Excellent communication and organisational skills.
• A calm, compassionate and professional approach.
• Flexibility to travel regionally.

Responsibilities

• Support safe and timely hospital discharges.
• Deliver high-quality, person-centred care.
• Support clients with daily living activities and promote independence.
• Build routines that improve confidence and wellbeing.
• Provide reassurance and continuity for clients and their families.
• Mentor and support new Support Workers.
• Work alongside Care Managers and Clinical Professionals.
• Ensure medication, equipment and care supplies are in place.
• Identify concerns and escalate appropriately.

Up to £29,000 DOE + £3,000 annual car allowance Paid mileage |Travel within London and surrounding areas This isn’t your average care role – be the reason someone gets to go home!

Travel is an essential part of this role: This is a regional role so you will need to be happy and able to travel across your designated region If you’re looking for a role based in one local area, this position is unlikely to be the right fit.
