Job summary

Debenham Group Practice, part of Suffolk Primary Care, is seeking part-time Care Navigators to handle patient contacts across phone, in person, and online. You will record requests in askmyGP and support the duty GP and broader team.

Experience in busy customer-facing roles is valued. NHS or primary care familiarity is nice to have, with opportunities for training and development within Suffolk Primary Care.

Qualifications

• Take patient contact by phone, in person and online.
• Record clinical requests accurately in askmyGP so the duty GP can review and act on them promptly.
• Take turns supporting the duty GP as part of the Care Navigation team.
• Signpost patients to other local services where that is the most appropriate response.
• Carry out reception duties and manage patient records.
• Support clinical and dispensary colleagues.
• Provide a courteous, efficient and helpful service to patients and colleagues.
• Support clinical and administrative teams with a range of tasks.
• Work across all sites as needed.
• Help keep the practice running smoothly across all sites.

Responsibilities

• Support the Care Navigation team and duty GP as needed.
