The Hungerford Surgery is seeking two motivated Care Navigators to join our friendly primary care team. You will be the first point of contact for patients, handling phone and reception duties while signposting to the right clinician or NHS service.

We value clear communication, calmness under pressure, organisation and teamwork. Full training will be provided, with 20–25 hours per week across four to five days, in a supportive environment, including training on IT systems such as SystmOne.

Qualifications

• Excellent communication and listening skills.
• Confident speaking to a wide range of people.
• Calm under pressure in a busy setting.
• Organised and able to prioritise workload.
• Good IT skills; experience with TPP SystmOne desirable.

Responsabilités

• Obtain the necessary information to signpost patients to the most appropriate clinician or service.
• Navigate patients to community, voluntary and other NHS services.
• Manage appointment bookings and enquiries.
• Provide high quality administrative support to our clinical and management team.
• Handle sensitive information with discretion and professionalism.
