Job summary

SSP Health at Carlisle Central Practice seeks a Care Navigator to be the first point of contact for patients, directing them to the right GP or community services and handling bookings for appointments and clinics.

The role covers telephone, face-to-face and electronic enquiries, with liaison to GPs and clinicians, and the provision of patient information with a friendly, professional, and compassionate approach.

Qualifications

• GCSE English and Maths at grade C or above is essential.
• Prior GP practice experience preferred but not required.

Responsibilities

• First point of contact for patients.
• Handle telephone, face-to-face and electronic enquiries.
• Book patient appointments and clinics.
• Liaise with GPs, clinicians, administrative staff and patients.
• Provide information from the clinical team to patients.
• Help patients access GP services and community services.
• Refer patients where appropriate.
• Carry out filing, coding and general admin tasks.
• Follow practice procedures and processes.
• Provide a friendly, professional and compassionate service.
