Job description

Active Care Group’s Care in the Home division continues to grow, ensuring we provide safe, reliable, and outcome-based care to all clients through our exceptional support workers, clinicians, and management team.

We have an exciting opportunity for an experienced Care Manager with NVQ 4 or above in Health and Social Care to join our friendly and supportive team. This is a great time to join our Care in the Home division as we expand.

As Care Manager, you will oversee care packages within Shropshire, Cheshire, and surrounding areas. This role is highly rewarding as it enables clients to stay in their own homes and maintain as much independence as possible. The role involves regular travel to visit clients, so a driver with their own car is required.

Active Care Group has over 25 years of experience in providing high-quality complex care tailored to individual needs, including clinical, personal, social, and emotional aspects. We pride ourselves on extending our care to support the wider family and social networks, recognizing the broader impact of complex conditions.

What you'll be working:

  • Hours per week, mainly Monday to Friday, with flexibility required
  • Participation in an on-call rota during nights and weekends

What you'll be doing:

  • Managing care packages in your area
  • Collaborating with the Business Development Manager to review referrals
  • Producing assessment reports for care packages
  • Building strong relationships with clients, families, and external agencies
  • Ensuring high-quality, client-focused care
  • Supporting recruitment and training of Support Workers
  • Maintaining accurate and timely records

What you'll have:

  • NVQ 4 or above in Health and Social Care
  • Own car and valid driving license
  • Passion for delivering high-quality, client-focused care
  • Experience in care at Team Leader level or higher
  • Management experience is advantageous
  • Excellent communication skills, both verbal and written
  • Positive attitude and commitment to continuous improvement
