Job summary

Community Care Live is seeking a Personal Advisor to support care-experienced young people as they transition into independent adulthood. The role combines practical, emotional and social support within a hybrid, full-time framework.

You will manage a caseload of Children in Care and Care Leavers, develop and review Pathway Plans, advocate on their behalf, and help them access opportunities and services across housing, education, health and social care.

Qualifications

• Experience of working with young people.
• Strong communication and relationship-building skills.
• Commitment to achieving the best possible outcomes for care-experienced young people.
• Effective advocate, organised in your approach to case management and able to work collaboratively within a multi-agency environment.

Responsibilities

• Build positive relationships with young people and provide practical, emotional and social support tailored to their needs.
• Manage a caseload of Children in Care and Care Leavers, developing and reviewing Pathway Plans and advocating on their behalf.
• Help young people access opportunities and services they need to thrive across housing, education, health, social care and voluntary sectors.
• Collaborate with housing, education, health, social care and voluntary sector partners to secure suitable accommodation and opportunities for independent living.
