Job summary

Nottingham City Council is recruiting a Personal Advisor to join the Leaving Care Service in Nottingham. You will support care-experienced young people as they transition to independent adulthood, managing a caseload and developing Pathway Plans.

You will work in a hybrid model from Loxley House with partner agencies to help secure accommodation, access education, employment or training, and build life skills.

Qualifications

• You will hold a Level 3 qualification in Health, Social Care, Children and Young People, Education or Community Support
• Or you will have demonstrable experience working with children, young people or vulnerable individuals in social care, education or community settings

Responsibilities

• Manage a caseload of Children in Care and Care Leavers to support transition to independent adulthood
• Develop and review Pathway Plans and advocate on behalf of young people
• Collaborate with housing, education, health, social care and voluntary sector partners to secure accommodation and opportunities
