Job summary

Civic Recruitment Limited is recruiting a Personal Advisor for a 3-month contract with a Local Authority in Nottingham. You will support Children in Care and Care Leavers to transition to independent adulthood, managing a caseload, developing Pathway Plans, and offering practical, emotional and developmental support to help young people succeed in education, employment, housing, health and wellbeing.

Qualifications

• Educated to Level 3 in Health & Social Care or related field.
• Level 4/5 qualification desirable.
• Professional qualification in Youth Work, Social Care, Housing or related area desirable.
• Knowledge of safeguarding principles and legislation.

Responsibilities

• Manage a caseload of Children in Care and Care Leavers; develop and review Pathway Plans.
• Provide practical, emotional and advocacy support to enable independent living.
• Maintain regular contact with young people via home visits, meetings and outreach.
• Collaborate with social workers, housing providers and health professionals.
• Support education, employment and training opportunities for young people.
• Maintain accurate case records and ensure compliance with policies and legislation.
• Promote Equality, Diversity and Inclusion in service delivery.
