Bluebird Care Medway & Swale in Rainham, Kent is seeking an experienced Care Coordinator to join our office team. You will own care rotas, match carers with customers, and ensure safe, reliable care from teams who know them.

You’ll work closely with Care Assistants, customers and families, planning ahead, handling changes, and keeping precise records to support excellent service. Full-time, 28k-31.5k GBP, pension, healthcare, and induction are offered.

Qualificações

• You’ll have experience in care coordination and a safe, person-centred service.
• You’ll need strong organisation and communication skills, and knowledge of care regulations.
• A full UK driving licence with access to a vehicle is required.

Responsabilidades

• Plan and maintain care rotas at least four weeks in advance, matching Care Assistants with customers.
• Ensure appropriate travel time between visits and meet agreed visit times.
• Respond to daily changes and arrange cover for sickness/holidays.
• Coordinate new care packages and adapt rotas as needs change.
• Keep customers, families and Care Assistants informed when plans change.
• Maintain accurate electronic records and document important conversations.
