Job summary

Rainbow Care Solutions in Coventry is seeking a dedicated Care Coordinator to organise rotas, schedule visits and match carers to clients across Coventry and surrounding areas. You will liaise with clients, families and healthcare professionals to ensure safe, person-centred support.

The role requires hands-on experience in domiciliary care or care coordination, strong organisational skills, clear communication, and confidence with digital rostering and care planning.

Qualifications

• Experience in domiciliary care or care coordination.
• Strong organisational skills and ability to work under pressure.
• Clear and compassionate communication.
• Confidence using digital rostering and care planning systems.
• Holds a full UK driving licence and has access to a vehicle.
• Right to live and work in England.

Responsibilities

• Create rotas and update schedules to ensure safe staffing levels.
• Schedule and coordinate care visits.
• Match carers to clients based on skills, experience and needs.
• Liaise with clients, families and external professionals.
• Support carers with day-to-day queries and problem-solving.
• Ensure care plans and records are accurate and up to date.
• Participate in the on-call rota as required.

About the role:

Due to continued growth in our services, we are recruiting a Care Coordinator to help us deliver safe, person-centred care to our clients in Coventry and Warwickshire surrounding areas. You will be responsible for:

• Creating rotas and updating schedules to ensure safe staffing levels
• Scheduling and coordinating care visits
• Matching carers to clients based on skills, experience and needs
• Liaising with clients, families, and external professionals
• Supporting carers with day-to-day queries and problem-solving
• Ensuring care plans and records are accurate and up to date
• Participating in the on-call rota as required

About you:

• Has proven experience in domiciliary (dom) care and/or care coordination
• Is highly organised and able to work under pressure
• Communicates clearly and compassionately
• Is confident using digital systems for rostering and care planning
• Holds a full UK driving licence and has access to a vehicle
• Has the right to live and work in England
