Job summary

The AICS Group is seeking a dedicated Care Coordinator to liaise between clients, support workers and package managers. You will help ensure smooth communication, accurate documentation and effective shift coverage while supporting the care team.

You will participate in supervision and mandatory safeguarding training, with responsibilities including rostering, client visits, and on-call rotations to maintain high-quality care delivery.

Qualifications

• Proven ability to coordinate people and systems.
• Excellent communication with clients, staff and managers.
• Strong team player capable of working in a supportive environment.
• Willingness to cover shifts and be on-call as required.

Responsibilities

• Coordinate weekly with available Support Workers and update the internal scheduling system.
• Carry out client visits and provide feedback to Package Managers.
• Create rotas based on staff availability and confirm coverage for shifts.
• Monitor documentation and ensure all care plans and safety paperwork are up to date.
• Support reviews and MDT meetings and assist with referrals and forms.
