Job summary

Active Care Group is seeking a capable Care Coordinator to manage weekly care scheduling and client support, coordinating with care workers and internal teams. The role involves handling many calls, adapting to emergencies, and ensuring high standards of safeguarding and continuity of care.

You will work closely with the Team Leader to escalate critical issues, maintain regulatory compliance, and build strong relationships with clients and stakeholders.

Qualifications

• Previous experience as a Care Coordinator.
• Excellent customer service and communication skills.
• The ability to remain composed and professional in high-pressure situations.
• A flexible, dynamic, "can-do" attitude with a strong work ethic.
• The confidence to work both independently and as part of a close-knit, supportive team.

Responsibilities

• Co-ordinating and scheduling care each week, ensuring clients receive seamless support in their own homes.
• Handling a high volume of inbound and outbound calls to both clients and care workers.
• Reacting quickly to last-minute changes, emergencies, and staffing challenges.
• Keeping your Team Leader fully updated on any critical issues.
• Ensuring compliance with policies and procedures, with a sharp eye on safeguarding, continuity of care, and regulatory standards.
• Communicating clearly and compassionately, even when conversations are challenging or sensitive.
• Thinking on your feet, solving problems fast, and helping the team deliver under pressure.
• Competently represent the company and maintain/develop positive relationships with internal and external stakeholders.
