Job summary

Care Santé Limited in Nottingham is seeking a dedicated Care Coordinator to join our home care team. You will coordinate care calls, plan rotas, support staff, and act as a primary contact for clients to ensure high-quality, consistent care.

The role requires strong organisation and communication, a driving licence, and the ability to adapt to changing needs. You will work 37.5 hours a week, with participation in the on-call rota and additional on-call payment.

Qualifications

• Previous experience in health and social care (essential)
• Strong organisational and communication skills
• Ability to prioritise workload and respond to changing situations
• A supportive and team-focused approach
• A full driving licence and access to your own vehicle

Responsibilities

• Plan and schedule care calls and create efficient rotas
• Carry out care assessments and regular client reviews
• Supervise and support care staff, including supervisions and competency checks
• Assist with recruitment, including interviewing care staff
• Support with medication processes including MAR chart audits
• Complete ECM system reconciliations
• Act as a key contact for client queries and concerns
• Support development of training materials and content
• Deliver Care Certificate training following train-the-trainer approach
• Maintain training records and Nourish-compliance
• Participate in the on-call rota
• Support general office duties as required
