Job summary

Abbots Care is seeking a Care Coordinator to manage the organisation and delivery of care, mainly at weekends and evenings, for service users in their own homes. You will lead a team of care assistants, ensuring person-centred care and engagement with commissioners and local authorities.

Role involves rostering, reporting to senior management, and supporting teams on a rostered on-call basis, with a focus on maintaining capacity and high-quality outcomes for service users.

Qualifications

  • Experience coordinating and rota management.
  • Vibrant, self-motivated and passionate about making a difference.
  • Excellent verbal and written communication; comfortable on the phone and writing emails.
  • Experience in a fast-paced environment and flexible.
  • Strong attention to detail; a completer/finisher.
  • Team player with strong interpersonal skills.
  • Holding a full driving license and access to your own vehicle.

Within the role as the Care Coordinator, you are required to manage the organisation and delivery of care predominantly at weekends and eveinings. You will manage a team of care assistants to support our service users in their own home to provide person centred care. This role is more than just rota management. You will be a part of our operational team, who ensure that we provide outstanding quality care is delivered consistently to our service users.

The role
  • Rostering and daily management of Care Staff who are based in the community

Person Specification
  • Experienced Co-ordinator / scheduler doing coordination and rota management
