Job summary

Voyage Care is seeking a Care Coordinator in Caversham. The role focuses on coordinating care delivery teams and ensuring regulatory-compliant documentation. You will monitor live calls and respond to day-to-day operational needs, coordinating with families and external professionals.

The position requires health and social care experience, proficiency with MS Office, and strong administration and customer service skills. On-call duties and 37.5 hours per week are included.

Responsibilities

• Coordinate care delivery teams and ensure documentation for new people, discharges, and transfers is in place

• Coordinate with families, healthcare professionals and external agencies; handle enquiries, schedule appointments and meetings
• Maintain confidentiality and manage information securely
• Support management with care delivery planning and on-call requirements

Care Coordinator - Caversham - £15.15p/h

This role is for 37.5 hours per week. The successful candidate will also feature on the on-call rota.

As a Care Coordinator with us you'll play a pivotal role in ensuring the people we support are kept safe by ensuring our care delivery colleagues are deployed effectively and that all administration is undertaken in line with relevant regulations and legislation. You'll be responsible for the managing and coordinating of the care delivery team, you will play a key role in ensuring that the relevant documentation is in place for any new people we support, discharges and transfers. You'll conduct live call monitoring using bespoke software and be the first point of contact for day-to-day operational requirements of our care colleagues. You will also work alongside the management team to coordinate with families, healthcare professionals and external agencies, handle enquiries, schedule appointments and meetings and handle all information with confidentiality.
