Job summary

Responsibilities
- Coordinate care delivery teams and ensure documentation for new people, discharges, and transfers is in place
- Conduct live call monitoring using bespoke software and be the first point of contact for day-to-day operational needs
- Coordinate with families, healthcare professionals and external agencies; handle enquiries, schedule appointments and meetings
- Maintain confidentiality and manage information securely
- Support management with care delivery planning and on-call requirements
