Job summary

Voyage Care in Reading is seeking a Care Coordinator to support the DCA Office in Caversham. You will ensure safe care delivery by effectively deploying staff and managing administrative tasks in line with regulations.

You’ll prepare documentation, handle transfers, and monitor live calls to support day-to-day operations. The role demands Health & Social Care experience, computer literacy, administration skills, and a strong customer-service mindset.

Qualifications

• Experience in Health & Social Care and proficient in Microsoft Office.
• Experience in an administration role and delivering excellent customer service.
• Computer literate and able to coordinate with families and professionals.

Responsibilities

• Coordinate the care delivery team and ensure documentation is in place for new people, discharges and transfers.
• Conduct live call monitoring using bespoke software and be the first point of contact for day-to-day operational requirements.
• Coordinate with families, healthcare professionals and external agencies; handle enquiries and schedule appointments.

Care Coordinator – Caversham - £15.15 to £16.65 with overtime and evening/weekend enhancements (Ts & Cs apply)

We’re on the hunt for a Care Coordinator to join us in our DCA Office in Caversham.

As a Care Coordinator with us you'll play a pivotal role in ensuring the people we support are kept safe by ensuring our care delivery colleagues are deployed effectively and that all administration is undertaken in line with relevant regulations and legislation. You’ll be responsible for the managing and coordinating of the care delivery team, you will play a key role in ensuring that the relevant documentation is in place for any new people we support, discharges and transfers. You’ll conduct live call monitoring using bespoke software and be the first point of contact for day-to-day operational requirements of our care colleagues. You will also work alongside the management team to coordinate with families, healthcare professionals and external agencies, handle enquiries, schedule appointments and meetings and handle all information with confidentiality.
