Job summary

Norfolk and Norwich University Hospital is seeking a Cancer Care Navigator to guide newly diagnosed patients through holistic needs assessments and connect them with health, social care and charity services.

The role involves telephone and face-to-face contact, signposting, and working across the Information Pod, Cromer Centre and community outreach to support patients and carers in managing care.

Qualifications

• NVQ Level 3 in Care or Health and Social Care or NVQ Level 3 in Business Administration or equivalent experience.
• Current or significant previous experience as a Health Care Assistant or equivalent, or in a senior non-clinical role.
• Excellent verbal, telephone and written communication skills.

Responsibilities

• Carry out holistic needs assessments with newly diagnosed patients and when treatment changes.
• Signpost patients to internal and external support services, making referrals and helping with application forms.
• Hold telephone and face-to-face contact with patients, relatives, carers and health and social care professionals.
• Staff the Information Pod and Cromer Centre for drop-in advice and information.
• Support outreach services in the community.
• Support health and wellbeing events, internal and external.
• Make clinical visits to inpatients.
• Work with clinical teams, local charities and inpatient care areas.
• Rotate across information and support services and work from the Rouen Road office.
