Job summary

Royal Wolverhampton NHS Trust is seeking a Cancer Care Navigator to work with the urology patient group and the Clinical Nurse Specialist team. You will lead Holistic Needs Assessments, provide personalised information and signposting, and support patients from diagnosis through their cancer journey.

You will engage with patients via phone, in clinics and in the community, organise health and wellbeing events, and work closely with the wider MDT to improve pathways and self-management.

Qualifications

• Experience in patient-facing roles within cancer care is desirable.
• Ability to handle sensitive information with discretion.
• Strong communication and empathetic listening skills.

Responsibilities

• Conduct Holistic Needs Assessments (HNA) utilising the Somerset Cancer Register
• Provide tailored information, guidance, and signposting to patients and carers
• Deliver support through telephone, face-to-face, and community outreach settings
• Assist in organising and facilitating Health and Wellbeing events to promote self-management
• Engage with patients in clinical environments, including ward and outpatient settings
• Collaborate with multidisciplinary teams and external organisations
• Communicate sensitive and complex information with empathy, professionalism and confidentiality
• Attend local Bladder & Prostate Cancer support Groups
