Responsibilities

You will be working with a variety of disciplines within the multidisciplinary team to achieve best outcomes for patients and ensure independence and well‑being. The post holder will carry out a variety of health‑care assistant duties as part of the patient’s holistic care, particularly in the areas of personal care, pressure area care, palliative care, wound management, diabetes and other long‑term conditions. You will work as part of a team – under the guidance of registered nurses but also as an autonomous practitioner within the community; this may include home visits or seeing patients in clinic settings.
