Job summary

HCRG Care Group is seeking a Community Nurse Band 5 to join The Hollies team in Midsomer Norton. You will deliver high-quality, person-centred nursing care in patients' homes, working within national policies and the NMC Code.

You will collaborate with families and healthcare professionals, manage a caseload, and contribute to service development while maintaining clinical excellence and patient safety.

Qualifications

• NMC/1st level registered Nurse.
• Full UK driving license with access to a car for work.
• Strong communication and organisational skills.

Responsibilities

• Assess, plan, deliver, and evaluate nursing care in partnership with patients, carers, and the multidisciplinary team.
• Monitor patient progress and escalate changes in condition appropriately.
• Prioritise and manage a caseload in line with patient need.
• Maintain accurate and timely patient records using electronic and paper-based systems.
• Deliver evidence-based care and contribute to quality improvement initiatives and audits.
• Promote health, wellbeing, and independence for patients and carers.
• Participate in team meetings, handovers, and mandatory training.
• Support the learning and development of students and colleagues.
• Ensure safe use and maintenance of clinical equipment and stock.
