Job summary

Gloucestershire Health and Care NHS Foundation Trust is seeking committed Band 5 community nurses registered with the NMC or newly qualified. You will manage a patient caseload, provide holistic patient-centred care, and work within Integrated Community Teams across community settings.

Qualifications

• Registered with the NMC as a nurse.
• Must have or be eligible for a UK Nursing degree (BSc).
• Community nursing experience is desirable; newly qualified nurses welcome.

Responsibilities

• Assess patient and carer needs through strength-based assessment.
• Develop individual care plans with patients and carers; provide information and education.
• Regularly evaluate care plans and report clinical changes to the team.
• Provide skilled nursing care interventions and support to colleagues, patients and carers.
• Administer and monitor medications following policies and procedures.
• Contribute to workload prioritisation with a delegated caseload of patients.
