Job summary

NHS Jobs is seeking a Community Nurse (Band 5) to deliver high-quality, person-centred home nursing in The Hollies, Midsomer Norton. You will work with GPs and the MDT to assess needs, plan care, promote independence, and support positive health outcomes in patients' homes.

You will be part of a supportive team, contribute to service development, training, and continuous improvement, ensuring safety, infection control, and documentation standards are met.

Qualifications

• Registered nurse with NMC registration and UK practice standards.
• Strong patient care skills with compassionate approach.
• Excellent communication with patients, families, and MDT.

Responsibilities

• Assess and plan nursing care for patients in their homes.
• Monitor progress and escalate changes as needed.
• Maintain patient records (electronic/paper).
• Deliver evidence-based care and participate in audits.
• Support learning and development of staff and students.
