Job summary

HCRG Care Group is seeking an experienced Community Nurse (Band 5) to deliver high-quality nursing care at patients’ homes in Melksham. You will work within AfC terms, 37.5 hours per week, and contribute to a supportive multidisciplinary team under NHS Pension arrangements.

The role requires NMC registration, strong communication, and driving licence. You will engage in ongoing professional development and ensure compassionate, evidence-based care aligned with policies and standards.

Qualifications

• NMC/1st level registered nurse with current practice.
• Full UK driving licence with access to a car for work purposes.
• Strong communication and organisational skills.
• Proven ability to work both independently and in a multidisciplinary team.

Responsibilities

• Assess, plan, deliver, and evaluate nursing care in partnership with patients and carers.
• Monitor patient progress and escalate changes in condition.
• Prioritise and manage a caseload to meet patient need.
• Maintain accurate records using electronic and paper systems.
• Deliver evidence-based care and participate in audits and quality improvement.
• Promote health and independence for patients and carers.
• Participate in team meetings, handovers, and mandatory training.
• Support the learning and development of students and colleagues.
• Contribute to a safe environment through risk assessment and infection control.
