Job summary

HCRG Care Group is seeking a Community Nurse Band 5 to join The Hollies team in Midsomer Norton. You will deliver high-quality community nursing care, work with GPs and multidisciplinary colleagues, and contribute to service development and health promotion.

The role offers NHS pension, Agenda for Change terms, and access to learning and development through The Learning Enterprise. You will work 37.5 hours per week with possible additional shifts, depending on needs.

Qualifications

• NMC/1st level registered nurse and eligible to practise.
• Ability to work independently and as part of a multidisciplinary team.
• Strong communication and organisational skills; flexible approach.

Responsibilities

• Assess, plan, deliver, and evaluate nursing care in partnership with patients and the multidisciplinary team.
• Monitor patient progress and escalate changes in condition as needed.
• Maintain accurate patient records using electronic and paper systems.
• Promote health, wellbeing and independence for patients and carers.
• Support learning and development of students and colleagues.
