Job summary

HCRG Care Group is seeking a Community Nurse (Band 5) to deliver high-quality, person-centered care to patients in their own homes from Melksham Community Hospital. You’ll work with GPs and a multidisciplinary team to assess needs, plan and deliver care, promoting independence and positive health outcomes.

The role offers 37.5 hours per week, autonomy within a supportive community team, and opportunities for ongoing professional development through the Learning Enterprise.

Qualifications

• You will demonstrate excellent communication, organisation and clinical skills.
• You must be a NMC/1st level registered Nurse.
• You will be eligible to work in the UK as a nurse.

Responsibilities

• Assess, plan, deliver, and evaluate nursing care in partnership with patients, carers, and the multidisciplinary team.
• Monitor patient progress and escalate changes in condition as needed.
• Prioritise and manage a caseload in line with patient need.
• Maintain accurate patient records using electronic and paper systems.
• Deliver evidence-based care and contribute to quality improvement and audits.
• Promote health, wellbeing and independence for patients and carers.
