Job summary

HCRG Care Group Holdings Limited seeks a Community Nurse (Band 5) to deliver high-quality, person-centred nursing care in patients' homes. You will work with GPs, families and a multidisciplinary team, assessing needs, planning and delivering care while promoting independence and positive health outcomes.

You will contribute to service development, CPD opportunities, and professional development within an open, supportive team culture. An UK driving license and IT proficiency are essential.

Qualifications

• NMC/1st level registered nurse with current PIN.
• Ability to work independently and in a multidisciplinary team.
• Full UK driving license with access to a car for work purposes.
• Confident IT skills including Microsoft Office (Outlook/Word/Excel).

Responsibilities

• Assess, plan, deliver and evaluate nursing care in partnership with patients and carers.
• Monitor patient progress and escalate changes in condition appropriately.
• Manage a caseload in line with patient needs.
• Maintain accurate patient records using electronic and paper systems.
• Deliver evidence-based care and contribute to quality improvement and audits.
• Promote health, wellbeing and independence for patients and carers.
• Participate in team meetings, handovers, supervision and mandatory training.
• Support the learning and development of students and colleagues.
• Ensure safe environment through risk assessment and infection control standards.
• Ensure safe use and maintenance of clinical equipment and stock.
