Five Rivers Health and Wellbeing Centre is seeking a Community Nurse (Band 5) to provide high-quality, person-centred care in patients' homes within Malmesbury and surrounding areas. You will work with GPs and the multidisciplinary team to assess needs, plan care, and promote independence.

Qualifikationen

• Registered nurse with NMC 1st level registration.
• Ability to work autonomously and as part of a multidisciplinary team.

Aufgaben

• Assess, plan, deliver and evaluate home-based nursing care with patients and carers.
• Monitor patient progress and escalate changes in condition promptly.
• Manage a caseload in line with patient need.
• Maintain accurate records in electronic and paper systems.
• Deliver evidence-based care and participate in audits and training.
• Support students and colleagues in learning and development.
