Job summary

HCRG Care Group is seeking a Community Nurse Band 5 to join Beacon House, Devizes. You will deliver high-quality, person-centred nursing care in patients’ homes, working within AfC terms and NHS Pension guidelines.

You’ll assess needs, plan and deliver care with colleagues, while maintaining records and supporting professional development. The role offers 37.5 hours weekly, with a strong focus on patient outcomes and team collaboration.

Qualifications

• As a Community Nurse, you will deliver high-quality, person-centred care in patients' homes and follow AfC terms and NHS Pension guidance.
• You will work in line with national/local policies, maintain records, and support continuous professional development.

Responsibilities

• Assess, plan, deliver, and evaluate nursing care with patients, carers and multi-disciplinary team.
• Monitor progress and escalate changes in condition as needed.
• Prioritise and manage a caseload according to patient need.
• Maintain accurate patient records in electronic and paper systems.
• Deliver evidence-based care and participate in quality improvement activities.
• Promote wellbeing and independence for patients and carers.
• Attend team meetings, handovers, supervision, appraisals and mandatory training.
• Support learning and development of students and colleagues.
• Contribute to safe practice through infection control and risk management.
