HCRG Care Group is seeking a Community Nurse Band 5 at Chippenham Community Hospital to deliver high-quality, person-centred nursing care in patients’ homes, working with GPs and multidisciplinary colleagues to assess needs and promote independence.

You will be a registered nurse with NMC, and the role offers autonomy within a supportive community team, opportunities for professional development, and access to NHS pension and wellbeing resources.

Qualifikationen

• NMC/1st level registered Nurse.
• Full UK driving licence with access to a car.
• Strong communication and organisational skills.
• Ability to work independently and within a multidisciplinary team.
• Proficient IT skills including Microsoft Office.

Aufgaben

• Assess, plan, deliver and evaluate nursing care in partnership with patients and carers.
• Monitor patient progress and escalate changes when needed.
• Prioritise and manage a caseload.
• Maintain accurate patient records (electronic and paper).
• Deliver evidence-based care and contribute to quality improvement.
