Job summary

HCRG Care Group is seeking an experienced Band 3 Community Healthcare Assistant to deliver compassionate nursing care in patients' homes and residential settings. You will work under registered nurses, supporting a range of delegated clinical activities and developing community nursing skills.

A seven-day service requires flexibility, and a driving licence with access to a car is essential. You will collaborate with District Nurses and Senior Nurses, maintain patient records, and participate in

Qualifications

NVQ Level 2 in Care or equivalent qualification is required.
Experience in health or social care is preferred.
Strong communication and interpersonal skills are essential.
Ability to work independently within a team-based clinical environment.

Responsibilities

Deliver high-quality nursing care to patients in their homes and care settings.
Carry out delegated clinical activities per training and care plans.
Support wound care, wound dressings, blood glucose monitoring, and continence care.
Perform phlebotomy following training and competency assessment.
Observe patients and report changes to the nurse in charge.
Organise and prioritise delegated visits for patient needs.
Maintain accurate patient records, including electronic records.
Collaborate with District Nurses and Senior Nurses and other team members.
