Job summary

HCRG Care Group is seeking a Community Nurse (Band 5) to deliver high-quality, person-centred nursing care in patients' own homes. You will work with GPs and MDT colleagues to assess needs, plan, and deliver care, promoting independence and positive health outcomes.

This role offers autonomous practice within a supportive community team, with opportunities for learning, career development and access to professional development through our Learning Enterprise.

Qualifications

• You will be an NMC registered nurse (1st level).
• Full UK driving licence with access to a car for work purposes.

Responsibilities

• Assess, plan, deliver and evaluate nursing care at patients' homes with MDT input.
• Monitor progress, escalate changes in condition when needed.
• Manage a caseload prioritising patient need and safety.
• Maintain accurate patient records electronically and on paper.
• Deliver evidence-based care and participate in quality improvement and audits.
• Promote health, wellbeing, and independence for patients and carers.
• Attend team meetings, handovers, supervision, appraisals and mandatory training.
• Support learning and development of students and colleagues.
• Ensure safe use and maintenance of clinical equipment and stock.
• Adhere to infection prevention and control standards.
