Job summary

HCRG Care Group in the United Kingdom is seeking a Community Nurse Band 5 to deliver high-quality, person-centred care to patients in their homes, aligned with national policies and the NMC Code.

You will work with patients, families, GPs and multidisciplinary colleagues, manage caseloads, maintain records, and contribute to service development while supporting learning and safeguarding.

Qualifications

• NMC registration with active practising status.
• Experience in community nursing or home care preferred.
• Able to work autonomously and as part of a multidisciplinary team.

Responsibilities

• Assess, plan, deliver, and evaluate nursing care in partnership with patients, carers, and the multidisciplinary team.
• Monitor patient progress, recognising and escalating changes in condition appropriately.
• Prioritise and manage a caseload in line with patient need.
• Maintain accurate and timely patient records using electronic and paper-based systems.
• Deliver evidence-based care and contribute to quality improvement initiatives and clinical audits.
• Promote health, wellbeing, and independence for patients and carers.
• Participate in team meetings, handovers, clinical supervision, appraisals, and mandatory training.
• Support the learning and development of students and colleagues.
• Contribute to maintaining a safe environment through risk assessment, incident reporting, and adherence to infection prevention and control standards.
• Ensure the safe use, maintenance, and management of clinical equipment and stock.
