Job summary

Jorvik Gillygate Practice in York is seeking an experienced Advanced Clinical Practitioner or Advanced Nurse Practitioner to join its established multidisciplinary team. The role offers autonomous clinical decision-making with scope for independent prescribing and management of acute and ongoing conditions.

You will assess, diagnose and treat patients, maintain records, and work with GPs, pharmacy and the wider team to deliver high-quality, patient-centered care in a busy primary care setting.

Qualifications

  • Registered healthcare professional with current NMC, HCPC or other appropriate registration.
  • Advanced clinical practice qualification at Masters level or equivalent.
  • Significant post-registration clinical experience.
  • Experience of autonomous clinical assessment and decision-making.
  • Independent prescribing qualification desirable.
  • Experience within primary care or urgent care settings, or similar.
  • Desirable heart failure interest or experience.
  • Experience supervising or mentoring clinicians or learners.

Responsibilities

  • Assess patients presenting with acute, undifferentiated and complex conditions.
  • Take comprehensive clinical histories and perform physical examinations.
  • Formulate differential diagnoses and clinical management plans.
  • Prescribe safely and within scope of practice.
  • Request, interpret, and act on diagnostic investigations.
  • Manage acute and same-day presentations.
  • Support management of long-term conditions.
  • Provide preventative and proactive care.
  • Undertake home visits when clinically required.
  • Refer to secondary and community services as appropriate.
  • Work closely with GPs and the multidisciplinary team.
  • Maintain accurate and contemporaneous clinical records.
  • Support patient self-management, health promotion and prevention.
  • Contribute to clinical audit, quality improvement and service development.

Job description

Jorvik Gillygate Practice is looking for an experienced and enthusiastic Advanced Clinical Practitioner (ACP) or Advanced Nurse Practitioner (ANP) to join our established multidisciplinary team. We are particularly interested in applicants who are qualified independent prescribers, although we would be happy to consider an experienced candidate who is currently completing, or committed to completing, their prescribing qualification. This is a varied and autonomous clinical role, working alongside our GP, nursing, pharmacy and wider multidisciplinary teams to assess, diagnose, treat and manage patients presenting with a broad range of acute and ongoing health needs., The successful candidate will work autonomously within their professional competence and scope of practice, with appropriate clinical support available., The successful candidate will work autonomously within their professional competence and scope of practice, with appropriate clinical support available. Advanced Practice

The role reflects the four pillars of advanced practice: Clinical Practice providing autonomous, evidence-based assessment and management of patients. Leadership contributing to clinical pathways, service development and multidisciplinary working. Education maintaining professional development and supporting the learning and development of colleagues and trainees. Research & Quality Improvement contributing to audit, evidence-based practice and service improvement.
