Story Terrace Inc. is hiring an Advanced Clinical Practitioner to deliver autonomous clinical care in a GP-led, primary care setting in Coventry. The role involves managing a range of acute and chronic conditions with independent decision-making and prescribing capabilities.
You will work 22.5 hours weekly (Mon-Tue) in a clinic, plus admin time, contributing to service planning and quality improvement while maintaining patient records and ensuring governance compliance.

The Advanced Clinical Practitioner will provide autonomous, expert-level clinical care, managing patients across a range of acute and chronic conditions. The ACP plays a key role in reducing unnecessary GP referrals, ensuring patients receive timely and effective care, and developing long-term relationships with patients to support optimal health outcomes. As an ACP, you will make independent clinical decisions, interpret test results, prescribe medications, and refer patients to specialists when appropriate.
This role is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and will require a Disclosure and Barring Service check for any previous convictions.
