Job summary

The Chislehurst Partnership invites an experienced Advanced Nurse Practitioner to join our friendly multidisciplinary team. You will work across three GP practice sites in Chislehurst, delivering expert assessment, diagnosis and treatment for acute and chronic conditions.

You will prescribe independently, lead clinical governance activities, and contribute to service development while maintaining top standards of patient care and safety.

Qualifications

  • Registered Nurse with current NMC registration.
  • Advanced Nurse Practitioner qualification and Independent Prescriber qualification.
  • Master's degree in Advanced Clinical Practice or equivalent experience.
  • Evidence of advanced clinical assessment and diagnostic skills.
  • Experience with EMIS Web and AccuRx in primary care.

Responsibilities

  • Maintain current NMC registration and practice within the Code.
  • Provide expert assessment, diagnosis and treatment for acute and chronic conditions.
  • Independent prescribing in line with evidence-based practice.
  • Lead service development and quality improvement within the practice.
  • Work across multiple sites within the Chislehurst Partnership and with the wider MDT.

Job description

As a valued member of our clinical team, you will work alongside GPs, Practice Nurses, Clinical Pharmacists, Paramedics, Healthcare Assistants and our wider Primary Care Network colleagues to provide outstanding care to our patients., As an Advanced Nurse Practitioner, you will work autonomously and collaboratively within the practice team, providing expert assessment, diagnosis and treatment for patients presenting with acute and chronic conditions. Responsibilities include: Assessing, diagnosing and treating patients with undifferentiated and complex presentations Managing acute and long-term conditions Independent prescribing in line with evidence-based practice Same-day and urgent care appointments Home visits where appropriate Providing clinical leadership and contributing to service development Supporting quality improvement and clinical governance activities Working closely with the wider multidisciplinary team and community services The successful candidate will work across our practice sites and play a key role in delivering safe, effective and person-centred care., The Advanced Nurse Practitioner is an autonomous practitioner responsible for the assessment diagnosis treatment and management of patients presenting with acute illness minor illness long term conditions and complex healthcare needs. The postholder will work independently and collaboratively with the wider multidisciplinary team to deliver evidence based care and improve patient outcomes. The role includes clinical leadership service development health promotion education and quality improvement activities.

General The postholder may be required to undertake other duties appropriate to the role and level of responsibility to support the effective operation of the practice.

Qualifications and experience
  • Registered Nurse with current NMC registration
  • Advanced Nurse Practitioner qualification
  • Independent Prescriber qualification
  • Master's degree in Advanced Clinical Practice or equivalent experience
  • Evidence of advanced clinical assessment and diagnostic skills
  • Evidence of continuing professional development, MSc in Advanced Clinical Practice
  • Qualification in Minor Illness or Minor Injury Management
  • Teaching or mentorship qualification
  • Leadership or management qualification
  • Training in long term condition management
  • Advanced life support certification
  • Primary Care or General Practice specific training, Experience working as an Advanced Nurse Practitioner or in an advanced clinical role
  • Experience assessing, diagnosing and managing patients with undifferentiated presentations
  • Experience managing acute and long term conditions
  • Experience of independent prescribing
  • Experience working autonomously and making complex clinical decisions
  • Experience undertaking clinical examination and diagnostic assessment
  • Experience of working within a multidisciplinary team
  • Experience using clinical systems and maintaining accurate patient records
  • Experience of delivering evidence based patient care
  • Experience contributing to quality improvement and clinical governance activities, Experience working within Primary Care or General Practice
