Issa Medical Centre in the United Kingdom is seeking an Advanced Nurse Practitioner to join a forward-thinking GP practice. You will undertake ward rounds in care homes, assess and manage conditions, and independently prescribe within guidelines.
You will work autonomously and with GPs, pharmacists, and the multidisciplinary team, delivering high-quality patient-centred care and leading clinical governance initiatives while maintaining detailed records.
We are seeking a highly motivated and experienced Advanced Nurse Practitioner (ANP) to join our friendly and innovative GP practice team. The successful candidate will play a key role in delivering high-quality patient-centred care, with a particular focus on undertaking ward rounds within our care home settings and supporting the management of patients with complex and long-term health needs.
Working autonomously and alongside GPs, nurses, pharmacists, and the wider multidisciplinary team, you will assess, diagnose, treat, prescribe, and manage patients, ensuring continuity of care and reducing unnecessary hospital admissions.
We are a forward thinking and friendly 2 GP partner training practice with 2 purpose built buildings in the heart of Deepdale and Fulwood.
We are a committed team consisting of GP's Advanced nurse practitioners, Clinical Pharmacists, Paramedic Practitioners, Physician Associates, Nurses and a supportive administration team.
